All About Mom Questionnaire

Students fill in the blanks with information about their mothers on this Mother’s Day quiz. They can talk to their moms or use their knowledge (and some creativity) to make the worksheet dull or fun. Either way, it’s from the heart.

I Love My Mother Because … Art

Students cut out the main part (the heart parts and the boxes in the middle) and choose four things they like about their mothers. They color each one, cut it out, and glue it to the squares inside the heart. Then, fold the heart towards each other so that when he opens the heart, the mother will see all the things she likes about him!

Children write or draw something special about their mother (or a special adult) on each puzzle piece. They then colored the puzzle letters and cut them out. Glue the pieces together to make a heart on construction paper, or place the pieces in an envelope to give as a gift.

How to Use Printables for a Mother’s Day Project

Here are some ideas to increase learning:

  • Mom show-and-tell: After completing the questionnaire, have each student present it to their mother. If you can’t have the moms in, ask each student to bring in a picture or share a picture of their mom. Students practice their presentation skills, and their mothers get to participate in the creativity of your class.
  • Make Mother’s Day crafts: Pair the students’ projects with a homemade bouquet, card, or other Mother’s Day craft.
  • Turn questions into art: After students have completed the questionnaire, ask them to use it to inspire a piece of Mother’s Day art. Can they make a sculpture of Mom’s favorite food? Maybe a piece of wall art with Mom’s favorite text? Use these questions and answers to inspire 21st century Mother’s Day gifts.
  • Share the love. If students have other women in their lives that they want to celebrate, use the blank heart puzzle to create puzzles for them all.
